Acupuncture Prevents Neuropathy During Cancer Treatment


Today's interesting post from healthcmi.com (see link below) looks specifically at how acupuncture can prevent neuropathy developing during cancer treatment. It's a startling claim and one that could be extrapolated to apply to other forms of neuropathy if you follow the logic behind it. However, acupuncture is still getting a bad press in some quarters and there's no doubt a little blind faith doesn't do any harm. However, if the science is sound then it could be an important breakthrough. The acupuncture terminology in the article may leave you a bit baffled, so you may need to go to a basic acupuncture site to discover what it all means but there's no harm in talking to your doctor or specialist about it and maybe a qualified medical acupuncturist too (to get a balanced view).

Researchers find acupuncture effective for alleviating chemotherapy induced peripheral neuropathy (CIPN) for patients with colon cancer. Neurotoxicity from chemotherapy may damage nerve fibers and lead to symptoms including pain, burning sensations, tingling, muscle weakness, balance disorders, paresthesia, dysesthesia, cold or heat sensitivity in the hands and feet, and a “glove and stocking” distribution of symptoms. The paresthesia is often experienced as a bilateral pins and needles sensation. Severity ranges from mild to debilitating. The researchers summarize their findings into three main benefits of acupuncture for patients with CIPN: 


Acupuncture administered prior to chemotherapy prevents CIPN.
Use of acupuncture during chemotherapy inhibits the progression of CIPN.
Administration of acupuncture after completion of chemotherapy eliminates or reduces persistent symptoms.

The research was conducted at the Cancer Treatment Centers of America at the Southwestern Regional Medical Center located in Tulsa, Oklahoma. The investigators applied Traditional Chinese Medicine (TCM) style acupuncture to 10 patients with chemotherapy induced peripheral neuropathy. They note that “all of the patients reported or had improvement in their CIPN grades after starting acupuncture.”

The researchers note that using customized acupuncture point prescriptions in the TCM style produces superior patient outcomes when the use of distal acupuncture points are employed. The researchers emphasize the need for customization of the acupoint prescriptions per each patient’s differential diagnostics for maximum clinical efficacy. In addition, they emphasize that distal acupoints “increase blood flow” and therefore have an especially important role in the treatment of CIPN.

The researchers note that CIPN symptoms are “typically dose-dependent”. As a result, CIPN may limit the amount of chemotherapy a patient can tolerate thereby interfering with the efficacy of anticancer therapy. This is particularly prevalent with drugs containing platinum including oxaliplatin, an important drug in the treatment of colorectal cancer.

Platinum based medications often have both acute and chronic adverse CIPN effects because they “produce direct damage to dorsal root ganglion (DRG) neurons.” The researchers add, “Cisplatin, taxanes, and vinca alkaloids have also been shown to exert damage to microtubules resulting in the disruption of axonal transport and thereby signal transduction in these nerve fibers.” The researchers note that “acupuncture for the treatment of CIPN has several advantages.” They note that “side-effects are minimal and it has been shown to be safe and effective in preliminary studies.” An additional benefit to antitumor therapy, they add that studies indicate that acupuncture regulates the immune system.

The researchers note that the data indicates that “TCM-based acupuncture, prescribed frequently and aggressively, and focused on the distal extremities may be effective for preventing and treating CIPN.” They add that an aggressive treatment would, at a minimum, employ the following acupoints:

The researchers note that aggressive acupoint therapy was not always needed to produce clinical results and some patients are too weak to receive aggressive therapy. All acupuncture needles used were between 1/2” to 1” in length and were between 36 - 38 gauge. For this investigation, DongBang brand acupuncture needles were used. For the Ba Feng, Ba Xie, and Jing-well acupoints only guide tube insertion of the needles was employed. Needle retention ranged between 10 - 45 minutes per acupuncture treatment session with an average of 20 minutes per session. Electroacupuncture was not administered.

The research indicates that an integrative medicine approach to chemotherapy for colon cancer patients has several benefits. Patients experience an improvement in the quality of life including better functionality and comfort. In addition, reduction or elimination of CIPN reduces symptomatic interference with the quantity and frequency of chemotherapy treatments. The researchers note, “In the current authors’ treatments, the goal is to combine Eastern and Western practices in order to best provide for the patient.”

Acupuncture Theory

The investigators provide TCM theory as it relates to CIPN. According to TCM principles, CIPN is caused by qi and blood flow abnormalities. They note that blood nourishes tissues and qi moves blood to the tissues. The researchers note, “CIPN results from the body lacking nourishment to the muscles (Blood) and the body not directing the Blood to the limbs (Qi).”

The authors provide TCM theory as it relates to colon cancer. Tumors are often a form of stagnation resulting from disruption of circulation. Heat stagnation is also common with colon cancer patients and is indicated by “an area that feels hot to the patient, red tongue, rapid pulse, dry stool, and constipation.” Toxin stagnation is also common and is often produced by exposure to chemotherapy. Phlegm or damp stagnation is common with fatty tumors and is often concomitant with the following symptoms: mucus in the stool, loose stool, slippery pulse, puffy tongue, a feeling of heaviness. Qi stagnation may occur and is characterized by: flatulence, bloating, irritability, moving pain, purple tongue. Blood stasis is indicated by sharp or stabbing pain, an immovable tumor, dark stools, and difficulty with defecation.

The authors note that their overall clinical impression of cancer patients is that they have a deficient constitution. They note, “the body is not strong enough to move energy to all of the tissues. Therefore, the energy stagnates. This is often the case with CIPN.” They add, “Acupuncture is an effective treatment option for CIPN because acupuncture not only stimulates the body to send blood flow to the site of the needle but also helps strengthen the underlying condition of Deficiency.” The authors note that by applying acupuncture to the extremities “it will act as a local therapy bringing Blood and nourishment to help treat the damage caused by chemotherapy.” Moreover, the authors note that “by strengthening the body against its underlying Deficiency, acupuncture helps restore balance to the body.”

Being Honest With Yourself Regarding Neuropathy Care Vid


More wise advice from Dr. John Hayes at beatingneuropathy.tv (see link below). This week he strongly advises patients to take a good look at their strategies for living with neuropathy. He's right that we're all searching for the miracle cure, or supplement to help ease our symptoms. That's understandable: there is no cure, so people naturally go looking for the next best thing. His advice is to be completely honest with yourself and look at your lifestyle and history too and share that honesty with your doctor. There's a much greater chance of improving your condition if you do that.

Having chronic pain means many things change, and a lot of them are invisible. Unlike having cancer or being hurt in an accident, most people do not understand even a little about chronic pain and its effects, and of those that think they know, many are actually misinformed.

In the spirit of informing those who wish to understand …
… These are the things that I would like you to understand about me before you judge me…

Please understand that being sick doesn’t mean I’m not still a human being. I have to spend most of my day in considerable pain and exhaustion, and if you visit, sometimes I probably don’t seem like much fun to be with, but I’m still me– stuck inside this body. I still worry about school, my family, my friends, and most of the time – I’d still like to hear you talk about yours, too.

Please understand the difference between “happy” and “healthy”. When you’ve got the flu, you probably feel miserable with it, but I’ve been sick for years. I can’t be miserable all the time. In fact, I work hard at not being miserable. So, if you’re talking to me and I sound happy, it means I’m happy. That’s all. It doesn’t mean that I’m not in a lot of pain, or extremely tired, or that I’m getting better, or any of those things. Please don’t say, “Oh, you’re sounding better!” or “But you look so healthy!¨ I am merely coping. I am sounding happy and trying to look normal. If you want to comment on that, you’re welcome.

Please understand that being able to stand up for ten minutes doesn’t necessarily mean that I can stand up for twenty minutes, or an hour. Just because I managed to stand up for thirty minutes yesterday doesn’t mean that I can do the same today. With a lot of diseases you’re either paralyzed, or you can move. With this one, it gets more confusing everyday. It can be like a yo-yo. I never know from day to day, how I am going to feel when I wake up. In most cases, I never know from minute to minute. That is one of the hardest and most frustrating components of chronic pain.

Please repeat the above paragraph substituting, “sitting”, “walking”, “thinking”, “concentrating”, “being sociable” and so on … it applies to everything. That’s what chronic pain does to you.

Please understand that chronic pain is variable. It’s quite possible (for many, it’s common) that one day I am able to walk to the park and back, while the next day I’ll have trouble getting to the next room. Please don’t attack me when I’m ill by saying, “But you did it before!” or Oh, come on, I know you can do this!” If you want me to do something, then ask if I can. In a similar vein, I may need to cancel a previous commitment at the last minute. If this happens, please do not take it personally. If you are able, please try to always remember how very lucky you are–to be physically able to do all of the things that you can do.

Please understand that “getting out and doing things” does not make me feel better, and can often make me seriously worse. You don’t know what I go through or how I suffer in my own private time. Telling me that I need to exercise, or do some things to get my mind off of it¨ may frustrate me to tears, and is not correct if I was capable of doing some things any or all of the time, don’t you know that I would? I am working with my doctor and I am doing what I am supposed to do. Another statement that hurts is, “You just need to push yourself more, try harder…” Obviously, chronic pain can deal with the whole body, or be localized to specific areas. Sometimes participating in a single activity for a short or a long period of time can cause more damage and physical pain than you could ever imagine. Not to mention the recovery time, which can be intense. You can’t always read it on my face or in my body language. Also, chronic pain may cause secondary depression (wouldn’t you get depressed and down if you were hurting constantly for months or years?), but it is not created by depression.

Please understand that if I say I have to sit down/lie down/stay in bed/or take these pills now, that probably means that I do have to do it right now – it can’t be put off or forgotten just because I’m somewhere, or am right in the middle of doing something. Chronic pain does not forgive, nor does it wait for anyone.

If you want to suggest a cure to me, please don’t. It’s not because I don’t appreciate the thought, and it’s not because I don’t want to get well. Lord knows that isn’t true. In all likelihood, if you’ve heard of it or tried it, so have I. In some cases, I have been made sicker, not better. This can involve side effects or allergic reactions. It also includes failure, which in and of itself can make me feel even lower. If there were something that cured, or even helped people with my form of chronic pain, then we’d know about it. There is worldwide networking (both on and off the Internet) between people with chronic pain. If something worked, we would KNOW. It’s definitely not for lack of trying. If, after reading this, you still feel the need to suggest a cure, then so be it. I may take what you said and discuss it with my doctor.

If I seem touchy, it’s probably because I am. It’s not how I try to be. As a matter of fact, I try very hard to be normal. I hope you will try to understand. I have been, and am still, going through a lot. Chronic pain is hard for you to understand unless you have had it. It wreaks havoc on the body and the mind. It is exhausting and exasperating. Almost all the time, I know that I am doing my best to cope with this, and live my life to the best of my ability. I ask you to bear with me, and accept me as I am. I know that you cannot literally understand my situation unless you have been in my shoes, but as much as is possible, I am asking you to try to be understanding in general.

In many ways I depend on you – people who are not sick. I need you to visit me when I am too sick to go out… Sometimes I need you help me with the shopping, cooking or cleaning. I may need you to take me to the doctor, or to the store. You are my link to the normalcy of life. You can help me to keep in touch with the parts of life that I miss and fully intend to undertake again, just as soon as I am able.

I know that I have asked a lot from you, and I do thank you for listening. It really does mean a lot.

TIPS FOR DEALING WITH PEOPLE IN PAIN

1. People with chronic pain seem unreliable (we can’t count on ourselves). When feeling better we promise things (and mean it); when in serious pain, we may not even show up.

2. An action or situation may result in pain several hours later, or even the next day. Delayed pain is confusing to people who have never experienced it.

3. Pain can inhibit listening and other communication skills. It’s like having someone shouting at you, or trying to talk with a fire alarm going off in the room. The effect of pain on the mind can seem like attention deficit disorder. So you may have to repeat a request, or write things down for a person with chronic pain. Don’t take it personally, or think that they are stupid.

4. The senses can overload while in pain. For example, noises that wouldn’t normally bother you, seem too much.

5. Patience may seem short. We can’t wait in a long line; can’t wait for a long drawn out conversation.

6. Don’t always ask “how are you” unless you are genuinely prepared to listen it just points attention inward.

7. Pain can sometimes trigger psychological disabilities (usually very temporary). When in pain, a small task, like hanging out the laundry, can seem like a huge wall, too high to climb over. An hour later the same job may be quite OK. It is sane to be depressed occasionally when you hurt.

8. Pain can come on fairly quickly and unexpectedly. Pain sometimes abates after a short rest. Chronic pain people appear to arrive and fade unpredictably to others.

9. Knowing where a refuge is, such as a couch, a bed, or comfortable chair, is as important as knowing where a bathroom is. A visit is much more enjoyable if the chronic pain person knows there is a refuge if needed. A person with chronic pain may not want to go anywhere that has no refuge (e.g.no place to sit or lie down).

10. Small acts of kindness can seem like huge acts of mercy to a person in pain. Your offer of a pillow or a cup of tea can be a really big thing to a person who is feeling temporarily helpless in the face of encroaching pain.

11. Not all pain is easy to locate or describe. Sometimes there is a body-wide feeling of discomfort, with hard to describe pains in the entire back, or in both legs, but not in one particular spot you can point to. Our vocabulary for pain is very limited, compared to the body’s ability to feel varieties of discomfort.

12. We may not have a good “reason” for the pain. Medical science is still limited in its understanding of pain. Many people have pain that is not yet classified by doctors as an officially recognized “disease”. That does not reduce the pain, – it only reduces our ability to give it a label, and to have you believe us.

CHOLERA AND HOMEOPATHIC MANAGEMENT

DEFINITION:-

     Cholera is an acute  infectious disease caused by vibrio cholera (‘coma vibrio’) and characterized by severe watery diarrhoea ’rice water’ fluid, severe dehydration and electrolyte depletion and features of acute circulatory collapse.

CAUSES AND TRANSMISSON:-

    Cholera is mainly caused by bacteria vibrio cholera (‘coma vibrio’).
-          The organisms are present in faeces of infected persons.
-          Infection spreads mostly through flies that contaminate food and water.
-          More commonly occur in places of pilgrimage or fairs where a large population assembles at a time without proper sanitary arrangements or proper food and water supplies.
-          Hot and humid season is suited for cholera.
-          Factors responsible for spread of cholera are food, fingers, flies, fluids and fairs.

PATHOLOGY:-

    The incubation period is short, usually 1-2 days. The organism rapidly multiply inside the lumen of the small intestine but do not invade the wall of gut. They produce an exotoxin that stimulates secretion of alkaline fluid from the mucosa. There is severe watery diarrhoea with consequent dehydration, depletion of sodium and potassium and acute circulatory collapse. Renal failure is likely to set in.

SYMPTOMS:-

    The classical variety is known as ‘Asiatic cholera’.
-          The onset is sudden with profuse watery diarrhoea.
-          The stools are dirty white in colour, the so called ‘rice water stools’.
-          Diarrhoea is associated with profuse vomiting but no pain or colic. These leads excessive loss of water and electrolytes and patient soon passes into stage of collapse or shock.
-          When dehydration sets, patient complains of increase thirst and restless. The tongue, skin is dry, eyes sunken, the cheeks are hollowed and expression is anxious.
-          Loss of sodium causes severe muscular cramps.
-          Loss of potassium causes extreme weakness and distension of abdomen.
-          Pulse is rapid, weak or thread in severe cases, urinary output falls markedly.
-          The patient may develop remittent type of fever or he may even have hyperpyrexia leading to coma and death.

DIAGNOSIS:-

    Microscopical examination of the stools will decide the diagnosis.

TREATMENT:-

    Confine the patient to bed immediately upon the sign of cholera, better give him no food whatever until the disease is under control, then only nutritious liquid food, ice may be allowed in moderate quantities. Disinfect all discharges; as soon as patient becomes cold apply artificial heat. Some advise the injection into the rectum of several pints of water.

PREVENTIVE METHOD:-

-          The hand must be washed clean before taking food.
-          The fly population should be destroyed. All foods must be kept covered and protected from flies.
-          Drinking water should be properly disinfected.
-          Proper disposal of faeces should be undertaken with strict measures.
-          Vaccination provides only temporary protection for a period not more than 6 months.

HOMEOPATHIC REMEDIES:-

1.      ARSENICUM ALBUM:

Sudden prostration with sinking of the vital forces, violent vomiting and purging. Surface of the body cold, internally the patient is very hot, burns like fire. Great anguish, restless, fear of death. Thirst drinks little and often. Dyspnoea, suppression of the urine. Cholera, with intense agony, prostration, and burning thirst. Scanty, burning, involuntary.

2.       CAMPHORA:-

 Asiatic cholera, with cramps in calves, coldness of body, anguish, great weakness, collapse, tongue and mouth old. Onset sudden and the disease rapidly becomes worse, even before the diarrhoea sets in the skin is cold and blue, yet does not want to be covered, extreme prostration and perhaps a nausea and vomiting, burning in the abdomen with great anguish.

3.       CARBO  VEG:-

In the later stages, with symptoms of collapse. The diarrhoea, vomiting and cramps have stopped and the patient lies as if dead, no signs of reaction. The face is pale, the lips blue, body cold, even the breath may be cold, the pulse is hardly perceptible, thread like. Wants to be fanned

4.       CUPRUM MET:-

When the diarrhoea, vomiting and cramps are severe. The body is cold; the skin is blue, terrible dyspnoea. Symptoms of paralysis of the organism. Gurgling and rumbling in the bowels, particularly when the diarrhoea is stopped. Cold water relives the vomiting. The best remedy for the cholera cramps, and vomiting also, when this is a prominent feature.

5.       HYDROCYANIC ACID:-

The only remedy when along with pulselessness, the respiration is slow, deep, gasping or difficult and spasmodic, taking place at long intervals, the patient appearing dead in the intermediate time. If any remedy is entitles to be spoken of as a charm. It is this. It would seem at times to restore animation to a corpse.

6.       SECALE CORN:-

 Cholera-like stools, with coldness and cramps. Involuntary stools; no sensation of passing faeces, anus wide open. Burning in stomach and abdomen; tympanites. Pale, pinched, sunken face. Retention, with unsuccessful urging. Burning sensation; better by cold; wants parts uncovered, though cold to touch.

7.       VERATRUM ALBUM:-

The attack begins with vomiting, purging and violent cramps. Terrible weakness, especially after vomiting. The patient is cold, covered with cold sweat, particularly on the forehead. Most important remedy for cholera. Pulse small, almost imperceptible. Thirst for cold water. Cutting pain in the abdomen. Cases marked by profuse vomiting and purging, with coldness indeed, but without deadly collapse and lividity.

Some of other drugs are kali bich, nux vomica, podophyllum, rheum, phos, and sulphur.

Neuropathy And Insomnia

We all know that a lack of a good night's rest thanks to pain has a knock-on effect during the day and over time can seriously reduce the quality of life. People with peripheral neuropathy experience a wide variety of symptoms including pain, numbness, burning and “pins and needle sensations.” Neuropathy which is caused by nerve damage, primarily affects the feet. Neuropathy foot pain can make it extremely difficult to fall asleep or stay asleep. The abnormal sensations associated with neuropathy along with heightened sensitivity to blankets or the bed sheets may contribute to sleep disturbances. During the day, people are often distracted from neuropathic pain. Daytime activities such as work, computer, hobbies such as gardening can help desensitize people from their pain. Adopting new habits to help you sleep

  • Make sure your bedroom is quiet, dark, and cool. Noise, light, and heat can interfere with sleep. Try using a sound machine or earplugs to mask outside noise, an open window or fan to keep the room cool, and blackout curtains or an eye mask to block out light.
  • Stick to a regular sleep schedule. Support your biological clock by going to bed and getting up at the same time every day, including weekends. Get up at your usual time in the morning even if you’re tired. This will help you get back in a regular sleep rhythm.
  • Avoid naps. Napping during the day can make it more difficult to sleep at night. If you feel like you have to take a nap, limit it to 30 minutes before 3 p.m.
  • Avoid stimulating activity and stressful situations before bedtime. This includes vigorous exercise; big discussions or arguments; and TV, computer, or video game use.
  • Limit caffeine, alcohol, and nicotine. Stop drinking caffeinated beverages at least 8 hours before bed. Avoid drinking in the evening. While alcohol can make you feel sleepy, it interferes with the quality of your sleep. Quit smoking or avoid it at night, as nicotine is a stimulant.

If sleep problems persist and interfere with your ability to function, it may be time to consult your doctor. Marijuana-infused gum enters clinical trials

Patients can smoke it, eat it and soon, they may be able to chew it.

Cannabis-focused biotech company AXIM Biotechnologies (AXIM) last week said it launched clinical trials on humans for medical cannabis chewing gum as a treatment for multiple sclerosis. The gum, MedChew RX, contains 5 milligrams of cannabidiol -- a non-psychoactive component of cannabis -- and 5 milligrams of THC -- a psychoactive cannabinoid.

Medical marijuana chewing gum "should allow for predictable and controlled release of the active ingredients," George Anastassov, chief executive of AXIM, said in a statement. It should not be socially stigmatizing, should have a pleasant taste and consistency and no undesirable side effects, he added. "Chewing gum meets all these criteria."

AXIM already has a cannabinoid chewing gum product, known as CanChew, on the market, however it does not contain THC and does not offer any medical claims.

The psychoactive component of THC is an effective way to treat patients with degenerative diseases, in addition to the medicinal properties of cannabidiol, which can be used to treat neurological conditions such as epilepsy. Additionally, the act of chewing helps preserve cognition and memory, as well as promotes overall oral health, Anastassov told MarketWatch.

While the current trials of the gum are only for MS patients, Anastassov says Axim hopes to expand the range of conditions it can be prescribed for.

"For multiple sclerosis, the market for treatment is quite large," Anastassov says. "Eventually, we will try to enlarge the conditions for this medication, such as pain." (More than 2.3 million people are affected by MS globally, according to the National Multiple Sclerosis Society.)

He says pain is one of the most predominant symptoms patients of a wide variety of diseases including MS suffer from globally, and there have been few game-changing drugs in the pharmaceutical market to help treat it.

There is no cure for MS, but current treatments work to speed recovery from attacks, slow the disease's progression and manage symptoms, according to the Mayo Clinic. The most common treatments used to manage symptoms are physical therapy, muscle relaxants and medications to reduce fatigue, depression and pain.

The Phase 1 trial of the gum is slated to begin in the second quarter of 2016. Though medical marijuana is legal in 23 states and the District of Columbia, it is classified on the federal level as a schedule I drug, which gives it the same illegal status as heroin and LSD and is considered to have no accepted medical use.

While the FDA has yet to approve any product containing cannabis, it has approved Marinol -- which contains dronabinol, a synthesized form of THC -- for anorexia, chemotherapy and AIDS patients. The FDA's website says it will continue to assess the effectiveness of marijuana for medical use, and will work with companies on medical cannabis research.

If the gum is approved by the FDA, it could be available in all 50 states, even if they have not legalized medical marijuana. "That's why we're going through the FDA," Anastassov says. "There's no ambiguity as to where it's legal."
